If you are having problems with your benefit coverage and/or claims, please contact the insurance company directly or call the District Insurance Office at extension 1015. In order for us to intervene on your behalf, we will require a signed Authorization Form due to the implementation of new HIPAA legislation surrounding privacy of an employee’s health information.
